Daily Symptom Assessment

Check Your Symptoms Using #CampusClear

Unless you are studying or working entirely remotely with no plans to come to campus this semester, you are required to complete a health self-assessment every day (including weekends)—even if you are not going to be on campus for the day or do not feel any symptoms. The #CampusClear mobile app makes it easy to monitor and report your symptoms, and it only takes a few minutes to complete.

If you are not feeling any symptoms, the app will clear you to come to campus and produce a unique daily “Fast Pass” screen that you may be asked to show at the start of classes or before activities on campus. Just as we will all hold each other accountable to wear masks and social distance, this is another way to show that we care about each other and are willing to take the extra steps to keep everyone in our community safe.

Using the app daily can help keep your health top of mind. Tracking how you feel every morning allows you to better detect when new or minor symptoms arise and take action immediately. Thinking about your health daily will also help you discern whether you’re experiencing ongoing symptoms due to a diagnosed condition, such as seasonal allergies or chronic migraines, or if you’re feeling new, unexplained symptoms that need to be reported.

Reporting Bugs:

We continue to work with the app developers to make adjustments to #CampusClear, and you may have noticed improved performance recently. This is still a very new technology and there are some issues that need to be resolved. Collecting problem reports in one place will help us work more efficiently with the developers. Use this form to share any problems that you encounter.

How It Works

If you have the smartphone application, you'll receive a notification every morning to remind you to complete the screening. Open the app or click the link to access the web interface and view your daily assessment screen. Please accurately report your symptoms in the app and follow the guidance on the screen. You will not be punished for reporting symptoms of COVID-19. Your health information will remain confidential, according to our privacy policy.

Step One

Select your affiliation by checking one of the following options on the screen:

  • I'm a Student
  • I'm Faculty/Staff Member

Step Two

Report if you don't plan to be on campus for the day by checking one of the following options:

  • I have symptoms, but I'm not on campus today.
  • I'm not on campus today, and I don't have any symptoms.

Step Three

Self-screen for the most common symptoms associated with COVID-19 and report them in the app. You should also report potential exposure or testing for COVID-19. Checking for a fever is one of the options, so be sure that you have a personal thermometer on hand for the assessment.

These are the options you can choose from:

  • I have no symptoms
  • I've tested positive for COVID-19
  • Within the last 24 hours, I was informed that I was within close contact (< 6 ft, > 15 minutes) of someone who tested positive for COVID-19
  • I was tested within the last 24 hours
  • Temp ≥ 100.4 D/38.0 C or Chills
  • New Cough
  • New Shortness of Breath
  • New Loss of Taste or Smell
  • New Sore Throat
  • New Congestion or Runny Nose (excluding seasonal allergies)
  • New Unexplained Fatigue
  • New Muscle and Body Aches (Unrelated to Exercise)
  • New Headache (Excluding Migraines)
  • New Nausea or Vomiting
  • New Diarrhea

Once you select all options that apply to you, hit the "See Results" button at the bottom of the page.

Step Four

Upon completion, the app will produce your Daily Fast Pass or give you instructions if you are not cleared for campus. Please have your Fast Pass ready when you come to campus so you can show it at the start of classes and organized activities. If you are not cleared for campus, follow the instructions on the screen.

Contact Tracing

In addition to reporting any symptoms you may be feeling, the app also asks you to report if you've had close contact with someone who tested positive for COVID-19 or if you've been tested for COVID-19. By logging this important information, you can help Loyola identify potential outbreaks, track diagnoses, and notify all community members who may have been exposed to the virus as quickly as possible.

We appreciate your support and cooperation to help make our return to campus safe!
